Medical Information Form

The Medical Information Form (MEDIF) must be signed by a qualified obstetrician and dated within 10 days of the departure flight. Passengers should contact the STARLUX Customer Service Center with the Medical Information Form (MEDIF) or email it to STARLUX Customer Service Center (customerservice@cs.starlux-airlines.com) 48 hours (2 business days) before flight departure and please use ''Pregnant Passengers-Medical Information Form'' as a subject. The request will be forwarded to the concerned department for further confirmation.

Other Notices

  • For flight safety and the health of the passenger, pregnant passengers need to inform the STARLUX Customer Service Center of the expected date of delivery along with other necessary information. It is strongly recommended that pregnant passengers carry their Medical Information Form with the required information while travelling.
  • Some countries place limitations on the entry of non-national pregnant women. Passengers are advised to check with the local consulate to confirm the specific requirements of the destination country.
  • Pregnant passengers who violate the regulations or submit false information may potentially cause significant disruption and expenses to the airline and other passengers. STARLUX reserves the right to recover its costs (including, but not limited to flight diversion, delay claims, etc.) through legal actions.
