  • Please attach following documents for identity verification:
    a. Completed and signed "Consolidation / Deletion of COSMILE Account Application" form.
    b. Passport copy which includes the photo and signature page.
  • If the applicant is aged between 2 and 18 years old, please also attach the following documents:
    a. Passport copy of the legal guardian.
    b. Supporting documents for the relationship between the applicant and his or her guardian. (e.g., Identity Card or Household Certificate)
  • The account deletion process takes about 7 working days. Please note that no further notice will be made after completing the deletion.
  • Applications with insufficient attachments will not be accepted.
