Flight Boarding Certificate

  1. Please attach following documents for identity verification:
    a. Completed and signed "Application for Flight Boarding Certificate" form.
    b. Passport copy which includes the photo and signature page.
  2. If you are a COSMILE member, please apply through “Flight Boarding Certificate”.
  3. This application is eligible only for international flights operated by STARLUX Airlines.
  4. This application is eligible only for requests made within one year of flight departure.
  5. The certificate will be provided in duplicate with the requested flight segment, and the processing fee is TWD 100 for each flight segment. (e.g., a round trip to Macau from Taipei shall be considered as two segments)

Flight Delayed/Canceled Certificate

  1. Please attach following documents for identity verification:
    a. Completed and signed "Application for Flight Delayed/Canceled Certificate" form.
    b. Passport copy which includes the photo and signature page.
  2. This application is eligible only for international flights operated by STARLUX Airlines.
  3. This application is eligible only for requests made within one year of flight departure.
