- Please attach following documents for identity verification:
a. Completed and signed "Consolidation / Deletion of COSMILE Account Application" form.
b. Passport copy which includes the photo and signature page. - If the applicant is aged between 2 and 18 years old, please also attach the following documents:
a. Passport copy of the legal guardian.
b. Supporting documents for the relationship between the applicant and his or her guardian. (e.g., Identity Card or Household Certificate) - Account consolidation means two accounts belonging to the same member. The system will keep only one record of the welcome mileage.
- Incomplete application will not be accepted.
